Enter an Illinois Zip code. Under “Select your Network” choose DeltaCare USA. At the time of enrollment, you need to enter the six digit Facility Code associated with the Illinois provider you selected. Should you need to update your DeltaCare USA facility ID, please login to your DeltaCare USA member portal or call Delta Dental Customer Service at 800-639-0327.

Pro Tip: The facility must be located in Illinois. Should you need to see a specialist, your primary dentist will provide a referral.

1. Click here for the provider search.
2. Choose “Find a Dentist”
3. Next to Choose your Network choose PPD Plus.
4. Enter your Zip Code and click “Find a Dentist”.
Pro Tip: The PPD network allows you to use any dentist you wish, but you will receive the greatest benefit by using a dentist who participates in the PPO network.
