DeltaCare DMHO Plan:
- At the time of enrollment, you need to select a primary dentist located in Illinois.
- If you did not select a provider, please email the Student Support Team with the 6-digit facility ID. Your facility ID must be selected by the 15th of the month to appear on the next month’s roster.
- Your benefits will only apply at your chosen dentist. If you need to see a specialist your primary provider will give you a referral.
- To find a participating dentist, please visit Delta Dental’s provider search. Under “Select your Network” choose DeltaCare.
- To use your benefits, contact the provider that you selected, and they will find you on their DeltaCare roster.
- You do not need a card to use your benefits. Your provider will find you on the roster with your name and date of birth.
- Once enrollment is activated, you can register with Member Connection to print a temporary ID card and view claims information. When registering on the Member Connection site, please use your University of Chicago student ID. If your student ID is less than 9 digits, please add zeros to the beginning. Registration on the Member Connection site is not necessary to utilize your benefits.
Delta Dental Customer Service:
MetLife Dental PPO Plan:
- Your MetLife Dental group number is: 249986
- Please allow 10 business days for your insurance to be activated.
- Once enrollment is activated, you can register with MyBenefits to print your ID card and view claims information.
- When creating a MyBenefits account, under “Access MyBenefits”, please enter “University of Chicago Student“. When registering your account, you will be asked for a “MetLife Assigned ID”. This is your University of Chicago student ID. If it is less than 9 digits, please add zeros to the beginning.
- Registration in MyBenefits is not necessary to utilize your benefits
- If you do not have an ID card, your provider can verify your coverage using your name, date of birth, and University of Chicago student ID.
- To find a dental provider, please visit MetLife.com. Next to “How can we help you?” select “Find a Dentist” and choose “PDP Plus”. Enter your Zip, City or State and select “Find a Dentist”.
- You will get the best benefit by going to an in-network provider. Out of network care is subject to a Maximum Allowable Charge.
- The MetLife Mobile App is available on the App Store and Google Play. Once downloaded, it can be used to find a participating dentist, view claims and to see your ID card.

Davis Vision by MetLife Plan:
- Your MetLife Vision Plan uses the Davis Vision network.
- A vision ID card is not provided. Your vision provider will use your name and date of birth to verify coverage.
- You may access information about your plan by creating an account on the MyBenefits website.
- When creating a MyBenefits account, under “Access MyBenefits”, please enter “University of Chicago Student“. When registering your account, you will be asked for a “MetLife Assigned ID”. This is your University of Chicago student ID. If it is less than 9 digits, please add zeros to the beginning.
